About the Role
This Primary Care Nurse position involves working collaboratively in a safe, caring, and respectful environment to deliver quality person-centered nursing care and defined nursing interventions in a Community and Treatment Room Setting.
The role requires a UK driving license to enable travel to GP practices within the HSCP area, and experience within General Practice/Primary care/Treatment room environment is essential.
Key Responsibilities
- Assess and deliver quality person-centered nursing care and defined nursing interventions in a Community and Treatment Room Setting.
- Work collaboratively in a safe, caring, and respectful environment with an integrated nursing team model and wider General Practice multidisciplinary team.
- Develop and maintain effective relationships with patients, families, and healthcare professionals to provide comprehensive care.
- Participate in the development and implementation of care plans, ensuring that patient needs are met and care is delivered in a timely and efficient manner.
- Contribute to the identification and reporting of any issues or concerns related to patient care, ensuring that these are addressed promptly and effectively.
- Stay up-to-date with the latest clinical guidelines, policies, and procedures, and apply this knowledge in practice to ensure high-quality care.
